Header rule to add Diversion header based on specific number in From field

I have tried to add a couple of header rules to my sip manipulation to insert a Diversion header when a user has his toll free number set a caller mask.  Carrier doesn't recognize the mask as being our DID number so will block unless I add a Diversion header with a valid DID for our account but still pass toll free number as calling number to the callee.

I am able to identify the Invite with the specific From number.  I am able to add a Diversion header to all INVITE's.  Can seem to understand how to add Diversion header only when specific From is used.
