Concurrent Manager issues after upgrade

We are getting below errors in concurrent manager  after we migrate middle tier R12.1.3(redhat 5- source server) to redhat 6( new target server) - Cloning and notes - Installation and Upgrade Notes Release 12 (12.1.1) for Linux x86-64 ( Doc ID 761566.1 ).
Also upgraded database version from 11.2.0.3 to 12.1.0.2 - following interoperatibility notes - Interoperability Notes EBS 12.0 or 12.1 with RDBMS 12cR1 ( Doc ID 1524398.1 )

Errors:
                   Process monitor session started : 13-JUL-2016 12:11:39
CONC-SM TNS FAIL
Call to PingProcess failed for FNDCPGSC
CONC-SM TNS FAIL
Call to StopProcess failed for FNDCPGSC
CONC-SM TNS FAIL
Call to PingProcess failed for FNDCPGSC
CONC-SM TNS FAIL
Call to StopProcess failed for FNDCPGSC
