Where is the association between an Environment and a Web Server made?

When a user logs into JDE they can select an Environment, if they enter an non-existent environment a list of valid Environments is presented.  What database table or CNC configuration interface do I find this association?

I'm asking because I have an environment that seems to have been orphaned.  I've have found some old incorrect OCMs and a Machine Identification setting that was missing, but I still don't have the environment showing/available. 

Another clue, the non-Web environment for the Fat Client development stations is not showing up either.

This environment was working at one point, but it was reconfigured for a different path code at some point.  I'm sure the "wiring" is still crossed somewhere.  I just can't find it.
