Hi Gurus, I am getting the following error, after I upgrade to 12.1.3

Hi Gurus , I have applied the 9239090 after horrible experiences in Windows 2003 server, After the patch I can't launch forms. I have followed document (Doc ID 1579805.1) but no use, all those solution did not work for me, can some body help or encountered this ?
