Fiscal/Anniversary date is invalid

When running payroll, I'm getting the Fiscal/Anniversary date is invalid warning. 

A form level error has occurred.

CAUSE: The DBA's Fiscal/Anniversary date is invalid for one of the following reasons:

  1. The Fiscal/Anniversary date type was not found in the UDC table (06/AF).
  2. The Fiscal/Anniversary date is blank and the DBA requires it to process annual limits or rollover.

RESOLUTION: Input a new value for the Fiscal/Anniversary date.

I have checked the date code, which is LSDT on the associated accruals and it is a valid date code from the UDC 06/AF - the UDC it points to on the form (Advance DBA).  My limit method is set to 3.
