Not able to add a request to existing request set

We are using EBS version 12.2.4

I tried to add a concurrent request to the existing request set. I have used define stage to add new request and gave a stage code and it is also showing in the link stage screen. But when I tried to run it as test. The program is not bringing up from the parent sequence. Attached is the screenshots. Please advise.
