Migrating11i Suppliers into R12.1.3.

We're migrating 11i into an already existing R12 EBS.  The supplier tables and structure have changed a lot.  Does anyone have a CV040 mapping document they'd be willing to share?  Or steer me to the appropriate documentation.  What I've been able to find is missing the mapping of the addresses.
