PO Approval Email Notification has been received by incorrect person

Hi,

We are on R12.1.3

Issue - PO Approval Email Notification has been received by incorrect person.

The person did not receive the email notification who was supposed to received. It was received by the irrelevant person who should not have received the same.

This is the first time it has happened and with only one notification. We are afraid of its possible re-occurrence. 

Can you please advise me on the problem area where I should look or is there any standard setup in Oracle?

Our Email Exchange team is saying that there is no issue from their side.
