HeuristicMixedException in WLS 12.1.3 cluster

We have a WLS 12.1.3 cluster with two managed servers, each on a distinct Windows Server VM.

We are getting HeuristicMixedException with the following stack trace:

javax.transaction.HeuristicMixedException: (WLStore_MYDOMAIN_JmsStoreDB_B, HeuristicHazard, (javax.transaction.xa.XAException))
