Issue with WINDOWS DB BUNDLE PATCH 12.1.0.2.160719(64bit):23530387

I have just applied the latest Windows DB Bundle patch on my 12.1.0.2 database and encountered error on the Database Vault component in some of the PDBs.  It resulted in the PDB to be opened only in the Restricted mode or effectively down to my application team.  I have logged Severity 1 SR but still not able to resolve after one day of troubleshooting.  Does anyone have same encounter for the patching?
