Advanced Pricing - Price List Bulk Loader Date Formatting

Hi...I'm trying to use the QP Interface tables to programmatically create a Price List with Price Breaks.

At this point I'm just trying to run the supplied SQL scripts to see how they build the information since I'm not quite following the need for having the child records. 

I'm on R12.1.3.

I'm looking at this note, specifically script QPBLKEX3.sql.

QPXVBLK: QP: Bulk Import of Price List - Troubleshooting - Including Sample Scripts (Doc ID 435467.1)

I've ran the 5 insert statements successfully and I'm running QP: Bulk Import of Price. 

It is finishing but nothing is creating. When I look at the Output File, it shows the following message:
