Lead Created with Different customer name for same Contact through Manual Lead Creation process

Dear All,

I have facing issue where Manual Lead Creation Process has been create multiple leads with same contact but these lead has different customer name. I have executed through campaign and activity. We dont  have any redundant data in audience.

Eventhough in Lead creation Application engine it is putting the Contact and Customer BO_ID based on State record and that state record has only one BO_ID as per Audience.

Please help me on same. Thanks in advance.
