ORA-29904 shown when auditing create session

Oracle RDBMS 12.1.0.2    

I am auditing create session whenever not successful, and have started to see a number of ORA-29904 Error in executing OCIINdexClose routine in dba_audit_session.  The action_name is LOGOFF, there is nothing in the alert log and not any trace files.  I know the application that his is coming from, the application owner sees nothing wrong at these times.  But they persist.

Any suggestions on next step troubleshooting to discover the underlying problem?
