Documentation for OTM 6.4.1 upgrade

Hello All,

We have OTM 6.2.7 with Oracle 11.2.0.4 database running. We want to upgrade to OTM 6.4.1 with 12c(12.1.0.2). So we have to upgrade DB as well as application.

Can somebody help with upgrade path and documentation which we can follow.
