Table or report that shows "who approved a PO" and "who created" a PO

Question on Purchase Order Approvers and PO originators.

1) Is there a STD report that would show  who created the PO and who approved the PO?

or

2) Is there a table that holds both this information  - Who approved the PO and who created the PO ?
