Concurrent request took a long time to run and couldn't be cancelled

We had an issue earlier this week with one of the concurrent requests.

The  "Pick Selection List Generation - SRS" request is scheduled to run every 15minutes throughout the day.  This request spawns the "Pick Slip Report" and it was the "Pick Slip Report" that was running for a long time, from 07.20am until 11.30am.  This started to cause a backlog of requests and others were taking a long time to complete.  We tried to cancel the request but couldn't, and even tried using sql command, but the request wouldn't cancel. 

What I'm wondering is whether anyone else has experienced a similar issue with requests taking a long time to complete and then not being able to be cancelled?  Maybe with this request or with a different request.....  What was the cause and how was it resolved?
