Cross Validation rule is stopping a code combination that doesn’t have anything to do with rule

We have a user that uploaded a journal entry in 12.2 but ended up with a cross validation error that has nothing to do with the rule. 

This rule is setup to not allow Company 001 from being booked to the state codes below as well as Comm accounts..

As you can see from the setup below, the code combination should NOT have violated this rule.  What could be the problem and how do we resolve it?
