call release after REFER transfer

We have a refer scenario working in nnECX640m4p1.tar. It stopped working for the remotely located SBCs last Tuesday, Sept 20, while it still works for the local based SBCs. Scenario goes as follows: 

INSIDE-realm -> SBC -> OUTSIDE-realm 

OUTSIDE realm sends a REFER message, SBC accepts it, and then triggers the INVITE cycle towards the new b-side in OUTSIDE realm. As soon as response is received, there is a refresh sent to INSIDE-realm, to setup call. After the first cycle of reINVITEs, INSIDE-realm side will send another reINVITE, which will trigger a reINVITE on the OUTSIDE-realm, remote side replies back, then SBC sends a 200 OK in the INSIDE-realm and INSIDE-realm a-side sends an ACK towards SBC. We do see the ACK message received in the logs, but after 30 seconds, we see the following in log.sipd:
