WebADI unsuccessful uploading file

At approximately 9:00 PM Monday 10/3 WebADI document creation in the Production, UAT, Test and Patch environments is failing. The template downloads, data can be entered, but the upload fails.
Error message: Server is unable to complete the upload. Please contact your system administrator for assistance.

We have one user in a different location who can upload successfully, but another user in the same location cannot. In our main location no one has been able to successfully create a document.  Any suggestions, ideas, assistance or just plain wild guesses will be appreciated. I'll update the thread if we find the issue.
