PR is going twice for approval to same approver.

When user creating PR,it is going twice for approver who has already approved it.We are using Employee/Supervisor hierarchy with hierarchy method.I have attached screenshot of the approval flow.Here the PR is going twice to Suresh Dhawal.What is the reason for this twice approval?

We are using Oracle APPS 11.5.10.2 version.
