Help needed in pivoting the output

Hello All,

Can someone help me out to understand how can i convert desired output from query which is coming in form of rows to columns. I tried Pivoting it, but saying 'non-constant expression is not allowed for pivot|unpivot values' Can we pivot varchar values ?

eg of the issue:

Once the output is generated, it will be in above form, but I want the output in below form. I tried pivot, but didn't work. Can someone help me out.

NOTES_DESC     CHANGE_DATE  USER_NAME  MEMBER   FISCAL_YEAR    FISACL_QUARTER  FISCAL_MONTH  PLANNING_ACCOUNT PRODUCT_LINE  ITEM
