warehouse Request at 250 for picking

Hi All

We have a strange issue . Just wanted to know if someone has come across this

We have problems with warehouse request going to 250 instead of 299. At this stage the task, suggestion everything gest closed but only rquest goes to 250

Any idea how can this happen

Let me know if you need detail process
