Positive Pay file not generating output

Hello,

I've created a custom RTF positive pay template.   It works fine when I run it using the BI Template Viewer.  When I run it as a concurrent request (Positive Pay File with Additional Parameters) for the same PAYMENT_INSTRUCTION_ID, a file is not generated.  How can I debug this program?
