How to get rid of HTML entity characters

We have recently upgraded from 9.0 to 9.2 and also changed how our text attachments are stored.  They are now stored as HTML.  We now have an issue that is not every time but unwanted characters show up on the report when using the media object system functions to get.  I have been unable to find anything yet on the Oracle site.  

We are getting &amp when an & is used and sometimes a &nbsp when there are several spaces.  Sometimes question marks when it looks like spaces but there may be other unprintable characters.  Has anyone else run into this problem?

Our tools version is also 9.2.
