sales order not created for specific customer through P4210

Hi,

When I enter sales order for a specific customer, fill the details and hit Ok, the order submitted successfully and system shows previous order number generated on header screen but the order is not found F4201/ F4211. It is happening only for 1 customer, only sometimes not always. So far, I have seen this issue 7-8 times in a month.

Any help would be much appreciated.
