fs_clone fails on missing custom.env file

We have upgraded to 12.2.5 running on AIX 7.1

We ran an fs_clone last night and the process failed.  From the error message it can't find the CUSTOM.env file.  Looks like the fs_clone did not copy or create that from the run filesystem.  This is not the first time we have seen this.  Not sure why this is happening or how to correct.

Any thoughts on what might be wrong here?

Stopping services on patch file system.
    Stopping admin server.

You are running adadminsrvctl.sh version 120.10.12020000.10

/apps/uat/fs2/inst/apps/UAT_srv1/admin/scripts/adadminsrvctl.sh[164]: /apps/uat/fs2/EBSapps/appl/CUSTOM.env:  not found
