Attachments on a form do not show text sequence number

I have a media object on a form and for this particular object media structure, it has two text media object Text1 and Text2 in F00165. Please see attached image.

As you can see in the image, the drop down list box does not have something like Text1, Text2 but blank.  Even though it's blank, it still have two entries in the drop down list box that I can choose from and the text attachment shows correctly for each one.  It's just that the drop down box does not have any label for Text1 and Text2

Do you know why? I test this locally so not sure if it's different on the server or not.
