Oct2016 PSU 12c OPATCHAUTO-72050: System instance creation failed; Topology creation failed

Hi

This is a three-node cluster, release 12.1.0.2; Oracle Linux 6 3.8.13-55.1.2.el6uek.x86_64.  CRS and most of the Oracle products are owned by user oracle.  There are two RAC databases and two services owned by user orasort.  We have just one group, dba, which is the primary group for both oracle and orasort.

When I added the orasort databases, I was able to register them and their services with Clusterware using srvctl from the orasort environment.  The databases were listed as cluster resources; I could see them when running command "/u01/grid/12.1.0.2/bin/crsctl status resource -t"

Today I wanted to apply the Oct2016 PSU 12.1.0.2.161018,
