Value Set created for UDA not working correctly

Hi All,

We have created a value set for all active employees, for use on our UDA pages but it is not working as expected.  There are 2 display issues:

Issue 1

Expected Result

In the field, enter the persons LastName and tab out of the field.  The system should display the first 15 results screen showing the list of people who match the chosen LastName and a link to select if there are more than 15 results to view.  Clicking the 'Next 15' link should display the next 15 results available.

Actual Result

In the field, enter the persons LastName and tab out of the field.  The system displays the first 15 results screen showing the list of people who match the chosen LastName and a link to select if there are more than 15 results to view. Clicking the 'Next 15' link displays the error:    "Error Page: You have encountered an unexpected error. Please contact the System Administrator for assistance".
