BI Publisher data model parameter

Recently when creating a data model using an SQL query with an input parameter, when pressing OK instead of seeing a check box for each parameter, I am prompted to "Please enter values for the bind parameter in query".

I have successfully created dozens of data models and reports with multiple prompts from SQL queries. The prompts work fine in SQL Developer and Toad.

Oracle BI Publisher 11.1.1.7.140715  (build:20140626.1140)

Anyone have any thoughts?
