Error "Value XXXX for the flexfield segment XXX does not exist in the value set <Value set>"

I am  having multiple issues once upgraded from 11i to R12.1.3. It started with an error on Intercompany Balancing error in the Concurrent program Submit accounting.

Steps

1. Submit Accounting produces the Error : Intercompany balancing was unable to generate the intercompany account. Please check the intercompany balancing rules setup.

2. Tried to add balancing segment value to Ledger : gave Error: "The Balancing Segment Value Assignments can only be confirmed after all legal entities have been assigned at least one value."

3. Tried to the same  BSV value  to legal Entity and that was successful.

4. Went back to Ledger and tried to add the same  BSV value  to Ledger, and it already showed up as existing !!! (Not sure how)
