Access to block zero in inode

Hi All

We had installed 4node RAC Oracle 12c GRID in RHEL 7 , all were working good . After a outage of clusternodes, One of the cluster node running on RHEL 7 Virtual machine is facing challenges for the CRS to come up. Rest 3 nodes of the cluster running fine.

Kernel/Tuning/OS Same across all nodes.

With further looking into the issue, when OHASD is starting it is recorded in /var/log/messages as below :

Nov 21 11:16:28 <hostname> journal: Oracle Clusterware: 2016-11-21 11:16:28.237#012[(17801)]CRS-8500:Oracle Clusterware OHASD process is starting with operating system process ID 17801

Nov 21 11:16:28 <hostname> kernel: XFS (dm-4): Access to block zero in inode 14661 start_block: 0 start_off: 0 blkcnt: 0 extent-state: 0 lastx: 18

Howdy, Stranger!
