R12 forms services started successfully, but forms page won't open

Hi All,

We are on EBS 12.1.3. For one of our EBS environment QA, we were able to start forms and other services successfully, but when clicking on a responsibility that opens a forms, the java console starts, then nothing happened. We have tried using different jre version such as 1.7.0_71 or 1.8.0_73, we also tried IE and firefox but got the same noresponse. We are using IE11. We can open up other 12.1.3 EBS environment fine, just not this qa.

Please help. Any suggestion is appreciated!
