Manually Entering Data into F08119

Has anyone tried to manually import your 1094C data into the F08119 for the 2016 reporting year and had success?  I did this for the 2015 reporting year, and was able to do it fine, but am having problems after taking ESU JL24022.  The records don't seem to be importing correctly, and it removes the person's name so the unique id fields are blank, and then none of the other records can be imported because there is a duplicate record for all of the other records because the first one gets imported blank.....  Any insight on this from anyone?
