How to add Opportunity Number -in LEAD Screen

We  are  using  in R12.1.3.  sales module.

  • In lead  menu sales man creating the Lead
  • Sales manager converts this lead to Opportunity.
  • Now Salesman want to query with LEAD number  and need to get LEAD & Opportunity Number.
  • Presently  in this screen  shows only 'Converted to Opportunity'

Any help to achieve this  is highly appreciated !!!
