Siebel Open UI: Toolbar and Settings icons are not displayed on refreshing the page

Hello there everyone,

One weird issues is bugging us from some days. In one of our dev environments User Icon which represent settings and Toolbar icons are not being displayed on page refresh. Some times they come and times they don't. As far as I remember they were working fine some time back but could not see any errors in console or file missing. Also checked the application level CCPageContainer and web templates all mappings are fine and same as other environments where icons are seen.

Anyone faced similar issue ?

Any trouble shooting or fixes methods available ?

Currently we are on IP2015... Using SUI Theme.
