BI Publisher eText template for Positive Pay file transmission to the Bank.

I am trying to create a Positive Pay Outbound eText template per the Bank's requirements.  Must have a single header line per file.  I am getting a one for one output regardless of how I set up the template tables.  I see either:

H = header  D = detail

Is there any way to produce a single header line.
