Incident Territory_Id is Null

Since we upgraded to R12.1.3, we're finding that the territory_id value in the cs_incidents_all_b table is sometimes null.  I have verified that there is a record in the jtf_terr_values_all table that fit the postal code from the service request.  Why would the territory_id be null?
