GI upgrade from 11.2.0.4 to 12.1.0.2 is failing with CLSRSC-180

GI upgrade from 11.2.0.4 to 12.1.0.2 failing while running rootupgrade.sh on the first node with "CLSRSC-180: An error occurred while executing the command 'clscfg -upgrade' (error code 111)". Following is terminal output.

stsqltst01(+ASM1):/home/oracle> sudo /grid/12c102/rootupgrade.sh

Performing root user operation.

The following environment variables are set as:

    ORACLE_OWNER= oracle

    ORACLE_HOME=  /grid/12c102

Enter the full pathname of the local bin directory: [/usr/local/bin]:

The contents of "dbhome" have not changed. No need to overwrite.

The contents of "oraenv" have not changed. No need to overwrite.

The contents of "coraenv" have not changed. No need to overwrite.
