Unable to Open Form in Planning

Hi Planning gurus,

I am experiencing an issue with one of our forms that won't seem to open. The form will load for between 2-5 minutes, and eventually an error will  occur that states the following:

Communication error with server. Callback failed for data entry page. [object Error]

After doing some research, the given resolution was to install a patch released 4 years ago (Patch 13588588:). We are on Version 11.1.2.1.600, which I believe  includes all the fixes of the patch that is recommended to install.

This form is an edited version of an existing form that is in our production environment. Some of the tests we ran:
