Migration from Grid Infra 11.2.0.4 to 12c latest version

Hi:

I am planning to migrate 11.2.0.4 Grid Infra and database from existing to a new server environment. What are the steps involved in migrating the whole environment to latest 12c version? Can i migrate 12c Grid Infra first and keep database running in 11.2.0.4 version before upgrading it to latest 12c? How are disks and disk arrays are handled as part of this migration? Could you please share valuable technical and example resources related to this effort.
