Custom Responsibilities and Custom Menus

What are your thoughts on every custom responsibility should be tied to a custom menu?  We currently don't have a standard and we have a variety of ways this has been done in the past.  (ex. Custom Responsibility tied to seeded menu and Custom Responsibility tied to custom menu).

We are planning an upgrade soon to 12.2.6 and we would like to use this opportunity to clean up our instance where necessary.  The standard was offered to have each custom responsibility tied to a custom menu and I wondered what is the norm... or is there a norm.

I know there are pros and cons to this approach.  I've very curious what is happening/suggested in the Oracle community.
