exit questionnaire and personalization

hi, we are currently reviewing our exit questionnaire. It is currently set up on employee self service, linking to a Special Information Type, set of questions which populates our core forms. We are finding that doing it this ways limits the length of question, how many and length of answers as i understand that we can only have up to a total of 240 characters worth of answers.

Is it possible to create a questionnaire using performance management and then link it using personalizations to the employee self service form, if so, when clicking create, what option would i pick?
