Unable to run and preview form

Hi,

I am creating a simple find/browse form TR 9.1.5.1.  I cannot preview the form in either run HTML Application and I get an error message when I'm in the FDA and selecting Preview->Run.  Can you advise what's missing?  This used to work just fine.
