OTL: Defaulting Overriding approver - inconsistent issue with "some" timecards going up supervisor c

We are on Oracle 12.1.3.

We have set an default overriding approver preference with eligibility criteria to "person" for a particular group of employees.  With this setting, when an eligible employee creates a timecard, the overriding approver automatically defaults.

The defaulting is working as expected.   However, we have had a few occurrences where an eligible employee's timecard will have the overriding approver saved in the timecard but when the employee submits the timecard, the workflow takes the supervisor chain approval type and sends the approval notification to their supervisor.

We had this issue in 2016 with the same setup but a different "overriding approver".  I logged SR in 2016 but didn't get anywhere since the issue is random and we were not able to duplicate the issue at will.   That overriding approver was made the actual supervisor --- which then eliminated this problem for her.
