Contract Revision request from lease center

Hi,

I was trying to submit a request for contract revision from lease center. I observed that it accepts the contract number and enters the request. Does it need any profile option to be set up for me to enter more details for this revision request.

Currently, it just shows request status as "Entered".

Any suggestion/reference would be helpful.
