user session interference

Hello,

we have an issue but it not repeated frequently, sometimes while the user working on his session another session for another user opened on front of him, and he can work on this session with all privileges of the other user, which is very critical issue although it is not repeated very often. I wonder if any of you face the same issue and what causing it.
