Requisition Approval notification had mismatch with the Amount from Requisition Total

Hello Gurus,

We have a Requisition approval notification, which has a different amount when compared to the actual Requisition Total.

Say the Requisition total has $1000, its notification is showing $4000.

What could be causing this issue, how to debug?
