ACA - What would cause the top of the 1095C form to be blank?

We are on Version 9.2 and tools release 9.2. When we run the 1095C Print (R08119B) the Part I (top) of the form (employee name, address, employer address, etc) is blank. Part II and III are filled out (except missing SSN or DOB), and there is no addresses on the back of the form. I verified in F0116 that the test employee has an address. And the form says 2015. Am I missing an ESU or setup?
