Sync dates on IR and ISO in 12.1.3

Every now and then our users find instances where the need by date on an internal requisition does not sync with the scheduled arrival date on the internal sales order. We have POR: Sync up Need by date on IR with OM set to Yes. Like stated this is more of a random occurrence. Any thoughts on why this would happen? On what we can check to see if it is data??
